Having my child’s last name changed, what do I do?

So me and my son's mother were not together when my son was born. I was present for his birth, signed the birth certificate, and all of that. However he was given his mother's last name instead of mine. He's older now and while me and his mother are still not an item, we do have a good and healthy relationship. She has agreed to allow his last name to be changed to mine. However, I've not the slightest clue as to what to do to get this set into motion. My question is how do I go about this and what should I expect? I live in florida if that makes a particular difference.
